Requires a Master of Social Work and licensure as an LMSW or LCSW-C in Maryland. Candidates must have at least 2 years of experience, specifically within hospital and pediatric settings, and proficiency with Epic software.

Position Overview:This position works in the Pediatric ICU, providing discharge coordination amid relentlessly high demand. This position is critical to throughput in medical floors. Tasks include family meetings, care coordination, referrals, securing community appointments, and collaborating with community agencies, programs, groups, etc. The SW is the liaison between the team and the family for a fast-paced, high-stakes, sensitive, and intense set of scenarios and needs.
Assist in coping with the challenges of caring for a child, managing their illness, hospitalization or outpatient treatment plan Support families (children, parents, grandparents, siblings) encountering a crisis, experiencing grief and bereavement or adjusting to a diagnosis Help the family and child gain a better understanding of the illness and what to expect while receiving treatment Discuss illness and/or hospitalization with the child and their family Advocate for the needs of the child and family with the medical team (doctors, nurses, therapists) to promote family-centered care Develop pre- and posthospitalization care plans Promote a safe family environment Refer to community agencies for material assistance, including temporary lodging or resources needed to support a safe discharge plan
MS Social Work from an accredited school of social work. Must be licensed by the Maryland Board of Social Work Examiners at the LMSW or LCSW-C level. Must have Epic Experience Minimum of 2 years of experience Experience with Hospital and Pediatrics experience required.
