Conduct comprehensive patient assessments and implement individualized treatment plans within patients' homes. Coordinate care with interdisciplinary teams and supervise Physical Therapist Assistants to ensure high-quality patient outcomes.
Requires a Master's degree and a current, active Physical Therapist license in the state of employment. Must possess a valid driver's license, auto insurance, reliable transportation, and BCLS/CPR certification.

What You’ll Do Conduct initial patient visits, including comprehensive assessments and evaluations Develop and implement individualized treatment plans Supervise and collaborate with Physical Therapist Assistants (PTAs) Coordinate care with interdisciplinary team members Monitor patient progress and adjust care plans as needed Complete timely and accurate documentation Lead patient discharge planning and education Why Join Us

What You Bring Current, active Physical Therapist license in the state of employment Valid driver’s license, auto insurance, and reliable transportation Current BCLS/CPR certification Home health experience preferred, but not required
