  • Responsibilities: Provide direct patient care under the supervision of a registered nurse. Conduct assessments, monitor patient conditions, and communicate findings to the healthcare team. Assist with personal care activities if needed. Offer emotional support and companionship to patients and their families. Collaborate with other members of the interdisciplinary team to develop and implement individualized care plans. Maintain accurate and up-to-date documentation of patient care activities. Records clinical/progress notes in client/patient's clinical record.

Pay

$60,000-73,000

QualificationsQualifications: 1. Preferably one (1) year of experience as a Licensed Practical Nurse with Hospice experience. 2. Has completed a practical (vocational) nursing program and is licensed in good standing in the state of Colorado and who furnishes services under the supervision of a qualified Registered Nurse. 3. Must be able to communicate in the language of the client and document client related activity as appropriate 4. Possesses adequate and effective written and verbal communication skills. 5. Possess adequate and effective interpersonal skills.

Physical

Demands: While performing the duties of this job, the employee is regularly required to talk and hear. The position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift or move items weighing 50 pounds.

Travel

This position requires daily travel. Mileage is reimbursed.

Requirements

Must be a Licensed Practical Nurse in good standing in Colorado with a completed vocational nursing program. Preference is given to candidates with one year of hospice experience and strong communication skills.
