Responsibilities

  • Conduct comprehensive assessments of patients' physical capabilities, medical histories, and functional limitations to establish baseline data and formulate individualized treatment plans.
  • Develop and implement evidence-based physical therapy interventions aimed at improving patients' mobility, strength, balance, coordination, and range of motion.
  • Educate patients and their caregivers on proper techniques for performing therapeutic exercises, using assistive devices, and implementing strategies to prevent injuries and promote optimal physical function.
  • Monitor patients' progress, reassess goals, and modify treatment plans as necessary to ensure optimal outcomes and continuity of care.
  • Collaborate with interdisciplinary healthcare team members, including physicians, nurses, occupational therapists, and speech-language pathologists, to coordinate patient care and promote holistic rehabilitation.
  • Maintain accurate and up-to-date documentation of patient evaluations, treatment sessions, progress notes, and discharge summaries in compliance with regulatory standards and agency policies.
  • Provide emotional support and encouragement to patients and their families, fostering a trusting and therapeutic relationship conducive to effective rehabilitation.
  • Stay abreast of advancements in physical therapy techniques, technologies, and research findings, and integrate new knowledge into clinical practice to enhance the quality of care provided.
  • Adhere to ethical principles and professional standards of conduct, ensuring confidentiality, respect for patient autonomy, and adherence to legal and ethical guidelines.
  • Participate in ongoing professional development activities, such as continuing education courses, seminars, and workshops, to enhance clinical skills, maintain licensure, and pursue specialty certifications as appropriate.

Qualifications

  • Graduate of an accredited physical therapy program with a Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) degree.
  • Valid state licensure as a Physical Therapist, with current CPR certification.
  • Prior experience in home health, geriatrics, or rehabilitative care preferred.
  • Strong clinical assessment and critical thinking skills, with the ability to develop and implement individualized treatment plans based on thorough evaluations.
  • Excellent interpersonal and communication skills, with the ability to establish rapport with patients, caregivers, and interdisciplinary team members.
  • Compassionate, patient-centered approach to care, with a commitment to empowering patients to achieve their rehabilitation goals.
  • Ability to work independently and manage time effectively, prioritizing tasks and adapting to changing patient needs in a dynamic home health environment.
