Essential Job Functions and Responsibilities

Key Responsibilities Patient Care

Conduct comprehensive assessments of patients and families to identify home care needs, including physical assessments and health history. Reevaluate patient nursing needs regularly. Develop, implement, and update individualized care plans based on nursing diagnoses, patient goals, and evolving conditions. Administer prescribed medications and treatments. Provide education to patients and families regarding care and health management. Identify and implement discharge plans as part of the care process. Serve as Case Manager when designated by the Clinical or Nursing Supervisor, coordinating care for assigned patients.

Communication

Maintain accurate clinical notes and update the primary physician as needed. Collaborate with physicians to report changes in patient conditions and obtain necessary orders.

Additional

Duties: Arrange for equipment and services essential to patient care. Supervise, train, and evaluate home health aides. Why Join OPO Health? Requirements

Qualifications and Skills: Active and unencumbered California RN license. Valid California driver’s license and proof of auto insurance. At least one (1+) year of relevant nursing experience. Current CPR certification.
