Provide personal care, companionship, and ADL assistance to clients under the direction of a Registered Nurse. Document care provided and report changes in client conditions to the clinical supervisor.
Must be at least 18 years old with a formal certification training program and current CPR certification. Requires a valid driver's license, insured vehicle, and the ability to follow written care plans.

Job Summary Provides personal care services under the direction of the Registered Nurse or Therapist. The Aide is assigned to specific clients by the Registered Nurse or other appropriate professional who performs services for clients as necessary to maintain their personal comfort and well-being.
Provide companionship and, if authorized, transportation to clients according to the care plan. Knowledge and experience with Alzheimer's and Dementia. Assist client with general Activities of Daily Living (ADLs) such as bathing, meals, and safe ambulation/transfers. Report any changes in the client’s condition to the supervisor as outlined in the care plan. Document care provided and maintain accurate records for the Agency. Work collaboratively with the client’s care team and ensure effective communication. Promote client safety by following infection control practices and reporting unsafe conditions. Attend in-service programs to meet compliance and notify supervisors of any educational needs.
Successful completion of a formal certification training program. Be at least eighteen (18) years of age. Demonstrated ability to read, write, and follow a written care plan. Good verbal, written, and interpersonal skills. Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Current CPR certification.
