Salary: $55,000/year Clearbrook is a non-profit, social services agency specializing in providing innovative tools, services, and opportunities to individuals with intellectual and developmental disabilities such as Autism and Down Syndrome. The Behavioral Health Therapist is directly responsible for the behavioral health care for clients and their families/significant others and assisting in the development and evaluation of those services. The Behavioral Health Therapist reports to the Director of Behavioral Health.
Essential
Functions: 1. It is the responsibility of the Behavioral Health Therapist to maintain confidentiality with all clients and their families and maintain a professional image/role as a representative of the agency. 2. Adhere to all policies and procedures established for Clearbrook, the CMHC Program, and those of all governing bodies. 3. Provides for the assessment, treatment planning, therapy, discharge planning, psychological testing (if appropriately credentialed), and case management (if appropriate) of assigned clients and families. 4. Is responsible for the timely documentation of relevant information, contacts, and action taken in the designated clinical record. 5. Provides referrals to clients for other services within the Organization network, if appropriate, or to other outside services as needed.
