The Licensed Practical Nurse (LPN) plays a vital role in delivering safe, efficient, and patient-centered care across Community Health Net locations. This position includes a $3,000 sign-on bonus, payable in accordance with company policy and contingent upon meeting all employment requirements. LPNs may be required to float between CHN locations based on operational needs. Key Responsibilities Conduct and accurately document patient health histories and assessments Assist medical providers in delivering high-quality, patient-centered care Manage patient flow, including triage, follow-up care, and care coordination Coordinate referrals to specialists and community-based services Provide health education to patients and caregivers Collect and process laboratory specimens (e.g., blood, urine, sputum) Support infection control practices, safety standards, and appropriate use of supplies Assist with front office functions as needed (scheduling, patient discharge, etc.) Float between Community Health Net locations as required.
