Provide comprehensive healthcare services and coordinate patient care Assessing and treating patients with gastrointestinal diseases and conditions Work closely with a team of healthcare professionals Assess, diagnose, and treat residents' physical and mental well-being Conduct thorough patient assessments, including medical history reviews and physical examinations Order and interpret diagnostic tests Develop accurate diagnoses and treatment plans Prescribe medication and administer treatments Provide counseling to residents and their families Collaborate with the interdisciplinary team for seamless coordination of care Manage chronic conditions and monitor progress Make appropriate adjustments to care plans Commitment to person-centered care Create a safe and nurturing environment for elderly residents in the nursing home
Minimum 6 months of experience in long-term care or a skilled nursing facility Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license DEA License CPR BCLS/BLS certified Must hold a Master's Degree in Nursing Completed a degree from an accredited Nurse Practitioner program A year of experience in Gastroenterology Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided Must have great organizational and time management skills. Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
