Candidates must hold a Master's degree in a related field and an active, unrestricted state license such as LCSW, LPC, or LMFT. Preferred candidates have 2+ years of clinical experience, particularly in geriatric or long-term care settings.
Vitae Health is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ overall well-being and adjustment to medical and life changes. Key Responsibilities: Conduct comprehensive psychosocial assessments for residents upon admission and as needed Develop, implement, and monitor individualized treatment plans Provide individual, group, and/or family therapy sessions Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns Collaborate with physicians, nursing staff, social workers, and rehabilitation teams to coordinate care Participate in care plan meetings and interdisciplinary team discussions Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations Provide crisis intervention and behavioral management strategies when needed Educate staff and families on mental health conditions and effective communication strategies Ensure compliance with HIPAA and ethical standards of practice
