The Physical Therapist will evaluate patients' physical and functional abilities to develop individualized care plans and provide hands-on therapy to improve mobility and restore function. They will also educate patients and their families on therapeutic exercises and collaborate with healthcare teams for coordinated care.
Candidates must be a graduate of an accredited School of Physical Therapy and hold a current state license. A minimum of two years of experience in a clinical care setting, preferably in home health or geriatrics, is required.
Description Klarus Home Health is seeking a PRN / Per Diem Licensed Physical Therapist to join our team to providing PT services including assessment, treatment planning, implementation, and evaluation for our patients currently enrolled in our home health program in the Waxahachie, Texas / Ellis County and surrounding areas.

Must be willing to do a Saturday rotation approximately once every 4-6 weeks Evaluate patients' physical and functional abilities to develop individualized care plans. Plan, implement, and modify physical therapy treatment programs based on patients' medical referrals and assessments. Provide hands-on therapy, exercises, and techniques to help patients improve mobility, relieve pain, and restore function. Educate patients and their families on therapeutic exercises, proper body mechanics, and injury prevention techniques. Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic patient care. Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans. Participate in ongoing professional development activities to stay abreast of current trends and best practices in physical therapy.
