About the role

The Family Medicine Advanced Practice Provider is responsible for treating patients of all ages in a primary care setting. This includes coordinating care activities, scheduling diagnostic procedures, and documenting patient information.

The Family Medicine Advanced Practice Provider, (APP), Physician Assistant or Nurse Practitioner, is part of an interdisciplinary team and responsible for treating patients of all ages in a primary care setting.

The Family Medicine APP provides initial and ongoing assessment and treatment of the patient's medical, physical, and psychosocial status, including: Coordinate the activities of the primary health care team to ensure optimal patient care services; candidates must be licensed to care for all ages.

Coordinate and schedule laboratory studies, diagnostic procedures, and services such as labs, x-rays, and referrals.

Explain the necessity, preparation, nature, and anticipated effects of scheduled diagnostic and therapeutic services to patients and their families.

Document in the patient’s medical record all relevant data including but not limited to results of diagnostic tests, laboratory results; patient’s condition, and response to therapies/interventions.

Ability and willingness to work Monday – Friday 8a-5pm; provide up to 36 hours of direct patient care.

Excellent communication skills.

Works effectively in a team environment.

Requirements

Candidates must be licensed to care for all ages and possess excellent communication skills. The role requires the ability to work effectively in a team environment.
