The Primary Care Pharmacist evaluates and enters provider orders for appropriateness of therapy, resolves drug-related questions with prescribers, and documents all medical information into the patient’s electronic medical record using the VA’s VISTA/CPRS system. Responsibilities also include providing patient consultation, drug information consults to staff, maintaining records of controlled prescriptions, and ensuring compliance with VA guidelines.
Candidates must be a graduate of an accredited school of pharmacy with an R.Ph or Pharm.D degree, and completion of a PGY-1 residency is required. A minimum of one year of non-retail clinical experience in a primary care or ambulatory care setting within the last four years is necessary, along with maintaining an unrestricted state pharmacist license and current Basic Life Support/ACLS certifications.

Valor provides a full range of medical services to veterans through the operations of CBOCs, tailored to meet the specific needs of local VA medical centers. The Primary Care Pharmacist is responsible for the interpretation of medical orders and counseling patients on proper medication administration and storage. The Primary Care Pharmacist must have strong communication with fellow colleagues to ensure the expectations of Patient Aligned Care Team metrics are met.
This position is Part - Time 2 days a week.
Graduate of an accredited school of pharmacy with a R.Ph or Pharm.D degree Completion of PGY-1 residency Certification by applicable professional organization Minimum one-year non-retail clinical experience (three years preferred) as a pharmacist within the last 4 years in a related primary care or ambulatory care setting; government healthcare environment preferred. Specific requirements could vary based on individual VA contract. Must be credentialed and remain in good standing through the Veterans Health Administration Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency – AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract Current, unrestricted Drug Enforcement (DEA) registration Proficiency in written and spoken English Strong computer skills Energetic and optimistic demeanor Strong service mentality and a focus on achieving all aspects of defined service standards This is considered a safety sensitive position. Final compensation will be based on relevant factors including work experience, skills, certifications, and internal equity.
