The LPN provides home health care including wound care, medication administration, and IV therapy. They are responsible for monitoring vital signs and educating patients and their families to support independence.
Candidates must hold an active LPN or LVN license and be a graduate of an accredited practical nursing program. Reliable transportation is required for travel to patient homes, and previous home health or acute care experience is preferred.
We are hiring dedicated Licensed Practical Nurse (LPN) to join our home health team in Massachusetts. Key duties include wound care, medication administration, vital signs monitoring, IV Therapy, and educating patients/families, often working independent to support daily living activities and patient independence.
Active LPN or LVN license in the state of practice.
Graduate of an accredited practical nursing program.
Previous home health, acute care, or long-term care experience is often preferred.
Strong clinical judgment, independent decision-making abilities, and effective communication.
Travel: Reliable transportation to visit multiple patient homes per day
