Engage and assist homeless individuals in transitioning from temporary shelters or hotels into short-term and permanent supportive housing. Coordinate lease signings, document procurement, and maintain accurate case notes and HMIS data.
Requires a Bachelor's degree and at least two years of experience in social services, preferably working with homeless or high-risk populations. Must possess a valid driver's license, automobile, and vehicle insurance.
The Housing Navigators will engage and assist homeless participants identified by stake holders who are in hotels, shelters, and other locations into housing. This will include supporting individuals as they move into a short-term housing solution first and maintaining contact and engagement as required to assist the participant with transitioning from short-term housing to either permanent supportive housing or subsidized housing.
Bachelor’s degree plus two (2) years’ experience in social services (case management experienced preferred). Experience serving homeless, ex-offenders, individuals living with HIV/AIDS and/or substance abuse population is preferred. An automobile, valid Driver’s License and vehicle insurance coverage is required. Occasional late or weekend hours may be required.
