Position Overview We are seeking motivated Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our expanding mobile care team.
In this role, you will provide in-home and in-clinic visits, perform health assessments, manage chronic and acute conditions, and deliver advanced wound care.
This is an opportunity to practice meaningful, relationship-based medicine while working autonomously in a flexible, supportive environment.
Provide advanced wound care treatment for patients with complex or non-healing wounds, including thorough assessment, debridement, and individualized treatment plans.
Deliver mobile wound care services directly in patients’ homes or care facilities, ensuring comfort, accessibility, and continuity of care.
Diagnose and manage chronic and acute conditions using a comprehensive, whole-patient approach.
Accurately document all patient encounters, treatments, and progress in the EMR system in accordance with organizational standards.
Actively participate in care planning and coordinate with internal and external support teams to optimize outcomes.
Ensure strict compliance with Medicare, state regulations, and organizational protocols governing wound care services.
Other duties as assigned.

Must hold an active and unrestricted Nurse Practitioner or Physician Assistant license in Nebraska or Iowa. Requires a valid driver's license and reliable transportation for mobile patient visits.
