Community Health Nurse-CHRN

• The Community Health Nurse improves health outcomes by providing both clinical support and health education to staff, clients and community partners.
• The Fresh Start Connect Program specifically serves pregnant and post-partum persons with substance use disorder.
• They facilitate communication between clients, their families, and the medical community in order to improve maternal health outcomes.
• This role is instrumental in providing post-partum and pregnancy education and support as part of an interdisciplinary team.
• This position requires travel and home visitation with clients.
• This position is 85% travel within the DCS regions listed above and the mileage and travel expense will be reimbursed.
• The expectation of this position is that all client contact is face-to-face.
• This is a grant funded position that is currently funded through June 30, 2027.

Qualifications

Applicants must have be licensed as a RN in the state of Indiana. At least 1 year maternity experience required and substance use treatment preferred. First Aid & CPR certification is required and must be maintained. VOAOHIN will provide certification if not already obtained.
