Responsible for the comprehensive case management of post-transplant renal patients, including developing care plans and coordinating medical interventions. The role involves managing patient communications, handling prior authorizations, and collaborating with providers to ensure high-quality patient care.
Requires a graduate degree from an accredited nursing program (ADN, ASD, or BSN) and an active RN license in North Carolina. Candidates must be CPR and First Aid certified with a valid driver's license and reliable transportation.
Responsible for the case management of the post-transplant renal patients.
This position is designated as an on-site Registered Nurse, patient-facing role. In-office presence is an essential function due to the need for real-time collaboration with providers, direct interaction with patients and caregivers, and secure handling of health data incompliance with HIPAA.
