Job Summary Infinite Medical Group is seeking a dedicated Palliative Care Physician to provide comprehensive, person-centered palliative and symptom-management services to residents in long-term care settings. This role is ideal for a physician passionate about improving quality of life, guiding goals-of-care discussions, and supporting residents and families as they navigate complex medical decisions.
Responsibilities
Deliver palliative and supportive care services to residents in skilled nursing and long-term care facilities Conduct comprehensive assessments focusing on symptom management, functional status, and quality of life Manage complex chronic and advanced illnesses with a comfort-focused, patient-centered approach Lead goals-of-care and advance care planning conversations with residents and families Collaborate with interdisciplinary teams, including facility staff and primary care providers Develop, implement, and monitor individualized symptom-management and treatment plans Order and interpret diagnostic tests when clinically appropriate Prescribe medications and evaluate ongoing treatment effectiveness Educate residents, families, and facility staff on disease progression, treatment options, and care expectations Complete all required clinical documentation accurately and in a timely manner Maintain a compassionate, respectful, and supportive care environment
Requirements
Active and unrestricted MD license Minimum 2 years’ experience in palliative care in a hospital or nursing home–based program Certification in palliative care is a plus Board Certification or Board Eligibility in Palliative Medicine, Hospice & Palliative Medicine, Internal Medicine, Family Medicine, Geriatrics, or a related specialty Strong communication skills with the ability to navigate sensitive and emotionally complex situations Commitment to elder care, comfort-focused medicine, and interdisciplinary collaboration
