The Case Manager is responsible for conducting individual intake evaluations, providing counseling sessions, and participating in client evaluations and case conferences. They also coordinate referrals for family members and assess patients' continuing care needs.
Candidates should have an Associate's or Bachelor's degree in Human Services or a related field, with a minimum of one year of experience in addiction treatment. A valid driver's license in good standing is preferred.
Scheduled Hours: 32 Hours, 1st Shift Benefits that start day-1! The Case Manager Responsibilities Are: Obtains documents and all relevant intake material. Conducts individual intake evaluation and counseling sessions, Shares responsibility for education and other group programming activities. Participates in client evaluations and case conferences. Provides referrals for affected family members to self-help programs when applicable. Assesses patients’ continuing care needs, contacts appropriate aftercare resources and arranges for transfer or admission to appropriate rehabilitation facilities and/or liaison with self-help organizations. Coordinate client’s transportation to next level of care.
Associates or Bachelor’s Degree in Human Services or related field preferred; High School Diploma or Equivalent required. A minimum of one year related experience and/or training in the area of addiction treatment. Valid Driver’s License in good standing preferred. Pay Range: $21.77 - $26.54 The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
