Provide primary care services, including physical exams and diagnosis, to residents in skilled nursing and long-term care facilities. Collaborate with interdisciplinary teams to develop and adjust individualized care plans for patients.
Requires an active and unrestricted NP, PA, or Physician license in New York. Candidates must have at least 6 months of experience in long-term care or skilled nursing facilities.

Infinite Medical Group is currently hiring a Primary Care Nurse Practitioner/Physician Assistant (NP/PA) to provide on-site care to residents in long-term care settings. This is a part-time position ideal for a provider who values meaningful patient relationships and a collaborative care environment.
