Provide end-of-life care and hospice services to patients in home or community settings during on-call hours. Responsibilities include assessing patient needs, developing care plans, and supervising hospice aide staff.
Requires a current unrestricted RN license, a valid driver's license with auto insurance, and two years of RN experience. Experience in Hospice or Palliative Nursing is preferred.

Culture & Values
The On-call RN Case Manager is responsible for providing end-of-life care and hospice services to patients in their home or community setting. This includes intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care.
This role requires compassion, strong clinical judgment, excellent communication skills, and the ability to support patients and families through highly sensitive and meaningful experiences.
Part-Time. Mon-Fri 5pm-8am, Sat & Sun – 24 hours -
At least two weekends a month
