Provide supportive counseling, crisis intervention, and daily living skills training to individuals with psychiatric disabilities across multiple residential sites. Manage resident service plan goals, maintain regulatory documentation, and ensure a safe, clean living environment.
Provides supportive counseling, ADL training, crisis intervention and restorative services (when applicable) to people with psychiatric disabilities in residential programs in compliance with relevant regulations
Educational requirements: High School Diploma required
Experience required: At least 5 years' experience preferred in a mental health related field
Duties: And responsibilities:
1.
Manage a rotating schedule at all 24 hour supervised sites, location of coverage will be changing and determined by site need and management.
2.
Provide counseling, behavioral interventions, restorative services, assistance in negotiating community activities and crisis management services.
3.
Monitor, train and assist residents in all programs in household maintenance activities to ensure a clean, safe and homelike environment.
Directly participate in household maintenance activities when required.
4.
Provide coverage at all residential sites as needed.
5.
Provide training, supervision and assistance to residents in daily activities including food purchases, recreational activities, and meal planning and personal care activities.
6.
Assist residents in developing meaningful service plan goals and provide supports to assist residents in achieving their goals 7.
Conduct fire safety training activities and ensures fire drills are conducted in accordance with regulatory guidelines.
8.
Participate in all Medicaid billable activities under leadership of the Supervisor, and/or ensure that all documentation required for the particular program is current and in compliance with regulatory guidelines.
9.
Document interactions with residents in accordance with agency policies and standards.
10.
Meet weekly, or as scheduled, with assigned primary residents for counseling and to establish and review service plan goals.
11.
Maintain collateral contacts with outside service providers and participate in case conferences as scheduled.
12.
Assist residents in procuring and maintaining financial entitlements.
13.
Monitor medications as required.
14.
Provide 24-hour supervision to residents in congregate facilities.
15.
Provide transportation as needed using personal vehicle when agency vehicle is not available.
16.
Promptly report all emergencies, incidents, maintenance and safety issues to senior Counselor or Supervisor.
17.
Participate in team meetings as scheduled.
18.
Complete admission documentation post-intake for new clients 19.
Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress 20.
Complete QI Training during onboarding and participate in QI refresher training as needed.
21.
