Clarvida is seeking an ACT Nurse (RN) to join our Assertive Community Treatment (ACT) Team serving adults with severe and persistent mental illness. In this role, you will be a critical part of a multidisciplinary team providing intensive, community-based services designed to support recovery, stability, and independence.
ACT services are delivered where clients live, work, and spend time in the community. You will support a specialized caseload, helping clients achieve treatment plan goals while remaining integrated with family, friends, and community supports. This position includes use of a company vehicle for client transportation as needed and participation in a rotating on-call schedule, which provides additional earned PTO.
● Provide nursing services as part of the ACT multidisciplinary team ● Meet with clients in homes, community settings ● Conduct basic health assessments, including vitals and labs ● Support medication management and facilitate long-acting injectable medications ● Coordinate care with medical providers and psychiatric staff ● Support clients with severe and persistent mental illness and/or substance use disorders ● Participate in a rotating on-call schedule (approximately one week every 6-8 weeks) ● Provide client transportation using a company vehicle as needed ● Maintain accurate, timely, and compliant documentation
● Active Registered Nurse (RN) in the State of Louisiana ● Experience working with individuals with severe and persistent mental illness and/or substance use disorders ● Psychiatric nursing experience preferred ● Valid Driver’s License in good standing and current automobile insurance
