Full Time

Monday-Friday 8-4 - Flexible Schedule

On call rotation with bonus pay

Locations Covered: South Franklin County and Madison County

Mileage Reimbursement @ .72 per mile

Position Summary

We are currently seeking a dedicated and compassionate Registered Nurse Case Manager to join our Home Health team. The ideal candidate will embrace our mission and provide skilled nursing care while coordinating the overall care plan for hospice patients and their families.

Qualifications

  • Education: Graduate of an accredited nursing program with a current RN license.
  • Experience: Minimum of two years of RN experience required. Previous hospice or home health experience is strongly preferred.

Requirements
