Provide primary and consultative professional care to geriatric and seriously ill patients within home and facility settings. Collaborate with physicians to develop treatment plans and manage chronic diseases to improve patient quality of life.
Requires a Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant with a current state license. Must possess a valid driver's license and the physical ability to lift 35lbs and move intermittently.

Key Responsibilities Optimal Care Providers (Nurse Practitioners and Physician Assistants) provide Primary Care and/or Consultative Professional Care in a variety of care settings. The Provider will function in collaboration with Physicians according to a Collaboration Arrangement. The Provider's practice will focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.
Hours • 8: 00 AM – 5:00 PM, Monday through Friday
