About the role

The Community LPN provides nursing, direct-support, and care management services to individuals in a supportive housing program. They ensure medical needs are met within the community and provide health education to both patients and team members.

Overview

Join our team and be a part of something extraordinary as part of Bridgeway’s Community Support Services program. Community Support Services (CSS) / Supportive Housing Community Support Services (CSS) / Supportive Housing provide services necessary to assist residents achieve mental health rehabilitative and recovery goals across a series of wellness domains including employment, education, housing, physical and mental health. Services begin with the development of an individualized recovery plan through a comprehensive personalized needs assessment. Persons served in this program learn and practice skills in order to achieve community integration and remain living independently in the setting of their choice.

The work is sometimes challenging and always rewarding by engaging with persons we serve and co-workers as we all learn, grow, and thrive.

Responsibilities

As part of the CSS team, provides nursing services, direct-support services, care management, education and primary health care to persons served. Assures that medical needs are being addressed in the community. Provides support and education to other team members in nursing/medical treatment and health care. Nursing practice supervision is provided by the RN, or in group format quarterly by the Bridgeway Bridge-to-Wellness Medical Advance Practice Nurse.

Qualifications

High school diploma or equivalent, must have graduated from an accredited nursing training program and possess and maintain licensure, in good standing, in the State of NJ as an LPN One year of technical nursing training supplemented with two years of paid work experience in community nursing Valid driver’s license required with no more than one moving violation within the past 12 months (vehicle required) About Bridgeway Behavioral Health Services Bridgeway Behavioral Health Services facilitates, promotes, and fosters recovery from mental illness and co-occurring problems. Requirements

Candidates must have a high school diploma, graduate from an accredited nursing program, and hold a valid LPN license in New Jersey. A valid driver's license and vehicle are required, along with two years of paid experience in community nursing.
