  • Summary: As a member of St. James Hospital Department of Physical Therapy you will serve the Hornell and surrounding communities in both an inpatient and outpatient setting. You will work with people of all ages and abilities to maximize their quality of life. You will be responsible for the examination, evaluation, diagnosis, prognosis, interventions, and outcomes for patients with a variety of neurological, orthopedic, sports medicine, and vestibular conditions. Job Results & Essential Function Perform initial examination and evaluation upon receipt of the referral or through direct access. Develops and implements treatment plans with the inclusion of short and long-term goals and methods to achieve goals. Involves patients and caregivers in the development of treatment programs. Recommends additional consultation as appropriate and document in the patient chart. Treats patients using therapeutic activities/exercise, manual therapy including mobilization and manipulation, gait training, neuromuscular re-education, and modalities that include but are not limited to hot packs, cold packs, ultrasound, electrical stimulation, and Transcutaneous Electrical Nerve stimulation (TENS). Treatments may also include any skills/methods within the respective therapist’s training and the physical therapy scope of practice. Reevaluates patient's functional status progressively, to ensure that the treatment program goals are appropriate to the patient’s current needs; gradually advances the patient toward maximal performance and functional independence. Adjust treatment plan and goals accordingly. Maintains and completes documentation of patient's evaluations, treatment, progress discharge notes within 24 hours of the patient visit. Sends required information to appropriate professionals. Actively participates in rehabilitation team efforts to meet patient's needs, including, but not limited to counseling and teaching patients and their families, discussing changes and treatment programs with physicians, coordinating healthcare plans with other involved health professions (e.g. rehabilitation staff, nurses, social workers, etc.) and assist in discharge planning as appropriate. Is responsible for ensuring patient/resident/client safety with a focus on making safety-driven decisions. Charges patient appropriately for treatment provided. Carries out supervision of physical therapist assistants and physical therapy aides. Consistently strives to be a productive member of the institution and always puts time to good use. Delegates duties to support personnel as appropriate by training and law. Inspires confidence from patients, families, and staff by performing and communicating in a highly professional manner at all times. Participates in performance improvement activities. Attends mandatory hospital and department conferences and meetings. Strives to keep current on advances in the field, reads textbooks, consults with others for personnel and professional development. Provides in-services to fellow rehab staff, nursing staff including CNA's and others at department in-services. Assists with the supervision and guidance of new personnel, students, and volunteers as assigned. Reports progress or problems as appropriate. Assists with the implementation of new programs and procedures. Incorporates new ideas and procedures into the treatment program. Participates in a special hospital, departmental, and/or community projects including service on committees, staff/public/patient education programs. Assists to develop handout/home programs, evaluation procedures, etc. Assists in maintaining treatment area and equipment; submits requests for equipment recommendation to the director of rehabilitation services. Maintains department policies, procedures, objectives, performance improvement program, safety, environmental, and infection control. Is responsible for participating in department and/or system-wide team dynamic initiatives.

Requirements

Education & Experience Graduate of an accredited program approved by the American Physical Therapy Association with a minimum of a bachelor’s degree although a master's/doctorate degree is preferred. Licenses and/or Certifications New York State physical therapy license or eligible for licensing as limited by the New York State Physical Therapy Practice Act for the continuance of employment. Maintains a valid BLS/CPR Certification Physical Demands The ability to stand, walk and stoop most of the day. This position requires physical effort with patients having varying degrees of physical impairments. Must be able to lift up to 50 lbs. and occasionally perform team lifts with weights up to 400lbs. Equipment will be available to assist as needed. Language Ability Ability to read, write and interpret documents in English. Ability to effectively communicate to patients, family members, staff, and physicians. Math Ability Basic math skills and competency in creating figures such as proportions, percentages, rates, and/or ratios. Reasoning Ability Must be able to work in a harmonious manner with all other departments. Must be able to map out assignments and complete them in a timely manner. Working Conditions Work within the outpatient physical therapy department in the Medical Office Building 90-95% of the time. Physical Therapists will also work with patients at bedside in the Emergency Department and Medical/Surgical Department 5-10% of the time. There is potential for exposure to chemicals, communicable diseases, and medical waste. Airborne, contact, droplet, and isolation precautions may be required at times and you will observe universal precautions at all times.
