The pharmacist evaluates prescriptions for appropriateness and collaborates with healthcare professionals to provide clinical support for hospice patients. They are also responsible for ensuring regulatory compliance and managing the safe procurement and disposal of medications.
Candidates must hold a Bachelor's degree in Pharmacy (PharmD preferred) and a valid pharmacist license in the state of practice. Proficiency in pharmacy information systems and knowledge of CMS, DEA, and FDA regulatory standards are required.

Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support. Maintain up-to-date patient profiles, including medical history, and current medications (prescription and OTC). Provide clinical recommendations to physicians and hospice nurses regarding medication therapy changes and optimizations. Implement and enforce policies and procedures for the safe procurement, storage, administration, and disposal of medications. Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management. Perform specialized tasks that may include: Working in the Controlled Drugs (Narcotics) Room. Supervising automated machines to prepack medications. Supervising the preparation of IV medications. Supporting facility-based dispensing machines (e.g., Omnicell). Perform other pharmacy-related tasks and responsibilities as assigned.
Bachelor’s Degree in Pharmacy, Pharm D preferred License/Certification(s): current and valid pharmacist license in the state of practice Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
