The Hospice Transition Nurse acts as a resource for patients and referral sources to facilitate hospice admissions and sustain community partnerships. They manage patient care transitions from hospitals to home or hospice care, including coordinating medications and documentation.
Candidates must hold a current Registered Nurse (RN) license, a valid driver's license, and pass a background study. An associate degree is required, while a bachelor's degree and experience in hospice or home care are preferred.
