• Job Title: Community Health Worker-Endocrinology, Diabetes and Metabolism Department: Medicine | IM Endocrinology, Diabetes and Metabolism Community Health Worker serves as a liaison between health/social services and the community.
• The Community Health Worker will work with clients throughout the PowerBP+ intervention; they will work helping individuals, families, groups and communities develop their capacity and access to resources, including health insurance, food, housing, quality care and health information. will be a link for patients through Phase I and Phase II of the program, and will also team with IPE students to extend the CHW reach, and to expose students to the role of CHWs on healthcare teams.
• The CHW will address social needs through resource referral and connection with the Pathways Community Hub Model.
