The LPN Outreach provides personal and nursing care services to clients with mental health disorders within their homes and the community. Responsibilities include administering medications, performing assessments, and collaborating with a multidisciplinary team to implement care plans.
Candidates must be a Licensed Practical Nurse registered with the BCCNM and have graduated from a recognized Practical Nursing program. Required certifications include First Aid, CPR, Food Safe, a valid BC Driver's License, and access to a personal vehicle.
The salary range for this position is CAD $32.84 - $44.96 / hour Job Summary

Reporting to the Program Coordinator or designate and under the direction of the team leader, the Licensed Practical Nurse (LPN) Outreach works as a member of the multidisciplinary mental health services team in a collaborative nursing model with RNs and RPNs. The LPN Outreach performs assessments and plans, implements and provides personal and nursing care services to clients, both in their homes and in the community, who have a variety of mental health disorders. The LPN Outreach operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM) and in conformance with established policies, procedures, standards and philosophies of Fraser Health.
