Deliver person-centered, trauma-informed community support services to individuals and families in South Georgia. Manage electronic health records and update treatment plans to ensure alignment with delivered services.
Requires a bachelor's degree in social work, psychology, or a related field and one year of full-time experience in social services. Must possess a valid Georgia Driver's License and experience working with mental illness or substance use disorders.

Working Conditions: Must be physically and emotionally fit to work in a stress-filled environment to meet deadlines and deal with routine and crisis situations. May have extended periods of sitting, walking, and standing. Requires varying hours of duty to provide monitoring, educational and/or crisis handling, as well as travel to off campus locations in BHSGA or personal vehicle. Work Environment: Employee works in a locked facility with patients who can be a danger to themselves and others. He/she must be able to restrain patients in crisis and other situations, and to work shifts. Employees in this position may be exposed to virus, disease and infection from persons served or the working environment. Incumbents may be required to work extended periods of time at computer terminals. They may experience traumatic situations, including psychiatric and /or deceased patients. Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job: Must be able to lift fifteen to twenty-five pounds and endure extended periods of standing on a daily basis. While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, stooping and crouching as well as lifting and supporting the individuals served. En este puesto, desempeñarás un papel clave en el apoyo a personas y familias en todo el sur de Georgia con servicios de salud conductual compasivos y de alta calidad. El Gestor de Casos Móvil será responsable de brindar servicios de apoyo comunitario directo centrados en la persona, con información sobre traumas, orientados a la recuperación y basados en la evidencia. Revisará el historial médico electrónico del paciente para garantizar que toda la información personal y de contacto esté presente y actualizada en cada visita, que todos los formularios se completen y actualicen, que los servicios prestados en la visita estén incluidos en el plan de tratamiento y cuenten con las órdenes de servicio correspondientes, y que los planes de tratamiento se mantengan actualizados para garantizar su eficacia y la coherencia con los servicios prestados.
Licenciatura en trabajo social, psicología, salud mental, enfermería, educación, sociología, justicia penal, consejería, consejería de rehabilitación, orientación o un campo equivalente. Un año de experiencia a tiempo completo en la prestación de servicios sociales. Experiencia con personas con enfermedades mentales o trastornos por consumo de sustancias. Licencia de conducir válida de Georgia con menos de 5 puntos. Dominio de la comunicación verbal y escrita. Excelente gestión del tiempo, pensamiento crítico y capacidad para resolver problemas.
