• Purpose: - The Certified Home Health Aide (Hospice) at PARENTIS Hospice and Health s responsible for providing compassionate and personalized care to patients in their homes. The role involves supporting patients in managing their daily activities and ensuring their comfort and dignity during the end-of-life phase. The aide will work closely with a multidisciplinary team to deliver high-quality hospice care tailored to the needs of each patient.
• Key
• Responsibilities: - Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility. - Monitor and report changes in the patient's condition to the hospice team promptly. - Provide emotional support and companionship to patients and their families. - Administer prescribed medications under the supervision of a registered nurse. - Maintain accurate and timely documentation of patient care activities. - Ensure a safe and clean environment for patients by adhering to infection control protocols. - Collaborate with healthcare professionals to develop and implement individualized care plans. - Educate patients and families on hospice philosophy and care practices. - Respect patient confidentiality and uphold ethical standards in all interactions.QualificationsRequired
• Education: - High school diploma or equivalent. - Completion of a state-approved Certified Home Health Aide training program.

• Experience: - Minimum of 1 year of experience working as a Certified Home Health Aide, preferably in a hospice setting. - Experience in providing compassionate care to patients with terminal illnesses.
