The Community Health Worker will manage community health by building trusting relationships and advocating for members' needs. They will coordinate care with clinicians and provide education and resources to support health and wellness.
Candidates must have an associate's degree and three years of relevant experience in healthcare or social services. Fieldwork experience and excellent communication skills are also required.

1. Associate's degree and three years relevant healthcare/social services experience; or equivalent education/work experience. 2. Fieldwork experience such as home visits, conducting health screenings in the community, and/or navigating social services (like food, housing). 3. Excellent communication skills, both written and verbal. 4. Basic working knowledge of Microsoft Office applications, including but not limited to Word, Excel, and Outlook. 5. Must have reliable transportation for in-person meetings with members at home or other locations.
