The LPN provides in-home nursing care, administering medications and treatments under the supervision of a Registered Nurse. They are responsible for monitoring patient conditions, maintaining documentation, and collaborating with an interdisciplinary team to ensure patient dignity.
Candidates must be a graduate of an accredited LPN/LVN program with an active New Mexico license and CPR certification. A minimum of one year of nursing experience and a valid driver's license with auto insurance are required.

The LPN will provide in-home nursing care to patients under the supervision of a Registered Nurse. You will be part of an interdisciplinary team dedicated to ensuring comfort, quality of life, and dignity for every patient and their family.
Your Key Responsibilities Will Include: Administering medications and treatments following hospice protocols and nursing standards
Performing essential clinical tasks within the scope of LPN practice
Monitoring changes in patient condition and notifying the RN as needed
Contributing to the development and revision of care plans with measurable outcomes
Maintaining complete and timely documentation of all care activities
Offering emotional support to patients and loved ones
Participating in care coordination meetings and collaborating with the broader care team
Encouraging teamwork by supporting other staff as needed
Ideal Candidate Qualities: Graduate of an accredited LPN or LVN program
Holds an active LPN license in the State of New Mexico
CPR certification required
Possesses a valid driver’s license and auto insurance
Minimum of 1 year of nursing experience (hospice experience is a plus)
Strong interpersonal and organizational skills
A compassionate, professional approach to patient care
Comfortable with basic math and medication administration procedures
Additional Expectations: Respects all patient privacy and HIPAA guidelines
Stays current with continuing education and required training
Follows applicable healthcare regulations and best practices
May be included in an occasional on-call rotation
