Conduct intake and individualized needs assessments to develop service plans for clients experiencing homelessness. Provide referrals and support to help clients achieve housing stabilization and community integration.
The following education and professional experience are preferred: Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations in a related field.
