Location: Newark, DE

Position Summary

Center of Hope, part of Saint Francis, is seeking a Primary Care- Advanced Practice Provider to support continued primary care expansion and access initiatives across the Delaware market.

Full-service Family Medicine Center providing primary care to a medically underserved population in the Newark area, with a strong focus on the Latino community. The practice emphasizes cultural component care, supported by a bilingual care team committed to delivering inclusive, patient-centered services.

Practice Highlights: · Opportunity to build and grow a robust patient panel

· Marketing practice to promote growth

· Organizational support to drive practice growth and community engagement

· Integrated Epic EMR

· Strong referral network within Trinity Health Mid-Atlantic

Qualifications: · PA or NP from an accredited institution

· Eligible for Delaware licensure

· Professional working proficiency in Spanish

· Commitment to high-quality, patient-centered care

Role Responsibilities: · Provide comprehensive primary care for patients across the lifespan

· Assist with the diagnosis and management of acute and chronic conditions

· Emphasize preventive care and population health management

· Collaborate with interdisciplinary teams to optimize patient outcomes

· Maintain timely and accurate documentation

· Participate in quality and performance improvement initiatives

Schedule & Call

· Full-time schedule (36 patient-facing hours)

· Flexible scheduling options available

· On-call responsibilities
