The Health Coach provides health screenings and coaching using motivational interviewing to support participants at neighborhood Health Hubs. They collaborate with clinicians to design individual programs and facilitate referrals to primary, behavioral, and community health services.
Requires a High School Diploma or GED and two years of related work experience, along with a valid Driver's License. A degree in Health or Social Services and experience in chronic disease management are preferred.

$18.54/Hourly
This is a grant-Funded position funded until december 31, 2026.
