Provide spiritual care and counseling to hospice patients, families, and caregivers while coordinating with other spiritual resources. Document patient needs and participate in the development of problem-oriented care plans.
Requires an ordained minister, priest, rabbi, or certified lay person with at least one year of experience in healthcare or death and dying. A graduate degree in theology or divinity is preferred.
Responsible for the provision of spiritual care services to patients/families/caregivers, either directly or through the coordination of care with other spiritual counselors.
