We are seeking a Registered Nurse Case Manager (RN-CM) to provide coordinated medical and behavioral health case management services within a detention center based in Detroit.
The RN Case Manager collaborates with nursing staff, behavioral health providers, physicians, hospitals, community agencies, and interdisciplinary teams to ensure patients receive comprehensive, patient-centered care throughout their treatment and transition planning process.

Coordinate medical, behavioral health, specialty, hospital, and community follow-up care for patients requiring ongoing treatment and continuity of care. Review intake screenings, referrals, and clinical documentation to identify individuals needing enhanced clinical oversight and case management support. Assist with discharge planning, patient education, medication continuity, referrals, and connection to community resources. Support nursing operations during busy shifts, including assessments, clinical overflow assistance, emergency response support, and coordination of patient care activities. Collaborate with physicians, behavioral health providers, nursing staff, hospitals, and community agencies while maintaining accurate and timely clinical documentation. Ideal candidates have strong clinical judgment, excellent communication and organizational skills, and the ability to respond effectively during high-stress medical and behavioral health situations while maintaining professionalism, empathy, and confidentiality.
Active, unrestricted Registered Nurse (RN) license in the State of Michigan. Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) required. Current BLS certification required. Previous experience in correctional healthcare, behavioral health, emergency nursing, case management, or community health preferred. Strong communication, organization, critical thinking, and crisis intervention skills with the ability to work effectively in dynamic clinical environments.
