About the role

Provide and coordinate comprehensive rehabilitative care to patients of all ages through evaluation and implementation of treatment plans. Maintain documentation and collaborate with physicians and team members to achieve patient goals.

  • Min USD $34.34/Hr. Max USD $53.23/Hr. Overview ***Sign-on Bonuses up to $30,000.00**
  • Bachelor’s degree in Occupational Therapy. Licensed or license pending in the State of Illinois without previous disciplinary action in Illinois or other states. Current CPR card. Responsible for providing and coordinating comprehensive care to patients of all ages through the proper utilization of evaluation, planning and implementation of a treatment plan in accordance with established standards, policies, procedures, protocols and guidelines. Qualifications

Licensure/Certification/Registry: O.T. license in the State of Illinois, or license pending with an application for licensure on file with IDPFR and a letter of “Authorization to Work” from the State of Illinois. The maximum allowable time to practice in a license pending status/under a licensed Occupational Therapist is 6 months per licensing regulations. CPR Certification preferred. If applicable, must possess valid Illinois driver’s license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (5 year MVR will be required).

Experience

Experience with neurologic and orthopedic injuries desirable.

Responsibilities

Other Knowledge/Skills/Abilities: Basic Knowledge in occupational therapy practices in accordance with O.T. Practice Act. Universal Precautions Category I. Must possess valid Illinois driver’s license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required). Must show proof of automobile insurance. Provides rehabilitative care to patients of all ages. Evaluates treatment needs of assigned physician referred patients of all ages. Determines and implements plan to accomplish treatment goals within appropriate time frames. Provides appropriate feedback to physician, department, and other team members and maintains required documentation. Performs other related work as required or requested. Requirements

Requires a Bachelor's degree in Occupational Therapy and a valid Illinois state license or pending licensure. CPR certification and a valid driver's license with automobile insurance are also required or preferred.
