The Nurse Practitioner serves as the primary care provider for geriatric patients in private homes and long-term care facilities. Responsibilities include conducting health assessments, developing personalized care plans, and coordinating care with other medical professionals.
Candidates must possess an Ohio NP license and a DEA registration. New graduates with backgrounds in home health, long-term care, or ER are encouraged to apply as no prior experience is required.
Nurse Practitioner - Geriatric Primary Care

This practice sees patients in assisted living facilities, independent living facilities, and private homes. They are hiring in the Youngstown area and will work with providers during the interview process to ensure a mutually-agreeable geographic coverage area.
This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of wherever they call home. They currently boast a team of more than 25 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, psychiatric care, post-acute care, and transitional care.
The nurse practitioner will serve as the primary care provider for a dedicated panel of geriatric patients located within a defined geographic territory within Youngstown. Patients are located in private homes (house calls), as well as long-term care facilities (ALFs, ILFs).
Providers see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs.
Dedicated schedulers put together daily schedules and routes for the providers to ensure efficiency and appropriate time for every visit.
The practice uses a mobile-based EHR with AI functionality to reduce administrative time.
On-call is phone triage only with very low volume. Providers rotate the on-call responsibility, with a shift every 12 weeks or less.
