About the role

Provide skilled complex care and symptom management to hospice patients and their families within the community and home environment. Collaborate with an interdisciplinary team to facilitate and oversee comprehensive care plans.

Overview

FirstHealth Hospice & Palliative Care is a 5 STAR rated non-profit hospice offering compassionate end-of-life care support.

As a Hospice RN you will have flexibility in providing skilled complex care to the hospice patient and family in the community.

The RN will work collectively with members of the interdisciplinary team to facilitate and oversee a care plan and symptom management needs in the home environment.

The complexity of managing and educating the hospice patient & family on advanced disease processes is a rewarding experience and opportunity to practice in a specialty area.

National Certification (CHPN) recommended after completion of at least two years of full-time experience as an RN in a hospice nursing practice.
