The Navigator RN provides expert nursing guidance and support to patients navigating complex healthcare journeys. Responsibilities include assessing patient needs, coordinating care across providers, and addressing barriers to care such as finances and transportation.

Job Summary and Responsibilities As a Navigator RN, you will provide expert nursing guidance and support to patients as they navigate complex healthcare journeys.Every day you will assess patient needs, educate on treatment options, coordinate care across multiple providers, and facilitate access to resources, acting as a central point of contact.To be successful, you will demonstrate strong clinical knowledge, exceptional communication and problem-solving skills, and a compassionate, patient-centered approach to ensure seamless and informed care.

Educate patients about their diseases, treatment, and side effects of treatment and how to manage them.

Provide clinical resources – where to seek a second opinion, treatment or testing that may not be available locally, and how to access reliable sources of information.

Assess patients for psychosocial distress and referral to a medical social worker or counselor if more intensive support is needed.

Address tangible barriers to care such as finances, housing, transportation, childcare, prescription costs, and much more.

They refer patients to the appropriate contact, such as a social worker.

Ensure any health information provided is accessible: this may include securing documents printed in the patient’s own language or arranging for a certified medical interpretation, either in-person or by phone/video conference.

Job Requirements Required 3 years clinical nursing experience and Registered Nurse: NE, upon hire and Basic Life Support - CPR, upon hire Preferred Bachelors Of Nursing BSN, upon hire and If working in Oncology, preference for oncology certified nurse (OCN), upon hire

Requirements

Candidates must have at least 3 years of clinical nursing experience, a valid Nebraska RN license, and BLS-CPR certification. A BSN and Oncology Certified Nurse (OCN) certification are preferred.
