Implements and executes physical therapy treatment plans to facilitate the rehabilitation of patients across various age groups. Responsible for evaluating patient progress, maintaining accurate records, and coordinating with interdisciplinary teams to maximize outcomes.
Requires a Bachelor's degree or higher in Physical Therapy and a valid license from the PT Board of the State of California. Must hold American Heart Association BLS certification and possess strong communication and clinical skills.
Education: Required: Bachelor’s Degree or higher level degree in Physical Therapy (PT) Preferred: Master’s degree or Doctorate degree in PT Licensure/Certification: Required: Licensed by the PT Board of the State of California; American Heart Association BLS Experience: Preferred: Physical therapy or healthcare experience Reports To: Manager-Physical Therapy Supervises: N/A Ages of Patients: Pediatric, Adolescent, Adult, Geriatric Blood Borne Pathogens: Moderate Potential Skills, Knowledge, Abilities: Ability to communicate with patients, healthcare professionals, and staff to ensure clear and accurate exchange of information, Ability to prioritize tasks and manage time efficiently to meet deadlines, Basic Physical Therapy Skills, Knowledge, skills, and ability to administer all department modalities, programs, and protocols appropriate to the age of the patients served, Written and verbal communication skills Essential Responsibilities 1. Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations. 2. Performs evaluation and established treatment plans based on age specific, cultural, and rehabilitative needs and in compliance of hospital licensing and accreditation standards and in compliance with the Physical Therapy Practice Act. 3. Provides effective therapy treatments. 4. Consistently and appropriately delegates patient treatment procedures and modalities. 5. Provides safe and comprehensive discharge planning. 6. Individual productivity meets department standards. 7. Provides thorough, timely, legible, and consistent patient care documentation. 8. Utilizes all resources to assist with patient flow as applicable (prints forms, schedules patients, etc.). 9. Provides effective education and mentoring for student interns as assigned. 10. Participates in special department programs. 11. Facilitates interdisciplinary communication and coordination to maximize patient outcomes. 12. Communicates positively, appropriately, and in a timely fashion to all communications including patients, co-workers, supervisors, other departments, physicians, etc.. 13. Maintains equipment supplies and treatment areas. 14. Performs other duties as assigned. Welcome to Eisenhower Health Careers!
