Provide comprehensive case management and coordination for families and individuals to support housing stability. This includes conducting intakes, linking participants to community services, and maintaining accurate data in the HMIS system.
Requires an Associate's degree in a human services field or at least 2 years of relevant experience in case management or counseling. A valid NYS driver's license and a reliable vehicle are mandatory for transporting participants.
PURPOSE: Provide case management and case coordination to families and single individuals living at Monica Place
1. Work with Rochester Housing Authority to complete intakes and initial screenings.
2. Provide case management and coordination service. This includes but not limited to consistent contact with head of household, entitlements assistance, medical, mental health, employment, literacy and higher education, legal services, safety planning, goal planning, etc.
3. Develop a working relationship with landlords and property management. Expected to establish consistent communication with the intent of supporting the household and addressing barriers to housing stability.
4. Develop YWCA and community partnerships that can be beneficial to program participants.
5. Enter timely and accurate data into HMIS (Homeless Management Information System) per coordinated access and YWCA policies.
6. Develop goals, document all contact, income changes and outcomes in HMIS.
7. Maintain household files per YWCA and funder standards.
8. Communicate and case conference regularly with supervisor, and other outside organizations working with program participants.
9. Link participants to appropriate services and activities within and outside the YWCA as identified in the program participant individualized service plan.
10. Assist program participants with the re-certification process as needed.
11. Case conference with Rochester Housing Authority, Partners Ending Homelessness, property rental manager, providers, and other community resources.
12. Participate in community workgroups/ meetings as it relates to the Continuum of Care and coordinated access.
13. Demonstrate understanding of and sensitivity to cultural differences, person centered practices and trauma informed care.
14. Demonstrate compliance with all appropriate HIPAA Privacy and Security Standards as defined by the YWCA of Rochester and Monroe County.
15. Attend monthly department and all staff meeting.
16. Perform other duties as requested.
Relevant background: Experience and Education Qualifications
§ Associates Degree in Counseling, Human Services, Sociology or Social Work. May consider at least 2 years’ experience in case management, crisis intervention or counseling.
§ Must have experience and proficiency in database management and Microsoft Office applications.

This is a grant position which may not exist beyond the duration of the grant period or any subsequent renewal of it. When the grant or any subsequent renewal ends, grant employees will be terminated and their positions will cease to exist. There is also no guarantee of continued employment for the duration of the grant.
A grant-funded employee may apply for and be considered for employment in another regular position in the same manner as any other applicant. If hired into a regular position, the employee’s salary from the grant position does not necessarily follow the employee into the new regular position.
Usual human services office working conditions prevail. Must be willing to travel unaccompanied to areas where the population served lives. Must be available to work 37.5 hours per week and the work hours may include evenings or weekends.
