Primary medical care, dental care, integrated behavioral health care, clinical nutrition, wellness programs, and chronic disease management are the foundation of its clinical programs. Founded as a free clinic in 1972, Open Door’s mission of building healthier communities through accessible, equitable, culturally competent health care has led to site and service expansions. Today, Open Door operates centers in Brewster, Mamaroneck, Mount Kisco, Ossining, Port Chester, and Sleepy Hollow, in addition to eight School-Based Health Centers in the Port Chester and Ossining School Districts, a mobile dental van in Mount Kisco, and a new dental practice in Saugerties, NY. Open Door is seeking a bilingual Spanish candidate for a full-time Medical Behavioral Health Clinician, LMSW, for its Family Medical Center located in Mount Kisco, NY. LMSWs who have or are interested in obtaining their CASAC are particularly encouraged to apply. The Behavioral Health Integration Specialist will report to the Director of Behavioral Health.

Responsibilities

Identify, screen, and triage patients for intervention daily Provide brief intervention treatment to patient and/or family Conduct screenings using appropriate screening tools to identify school readiness and monitor developmental milestones Document symptoms, problems, screening results, and interventions provided Develop treatment plan, goals, and objectives with patient Provide and facilitate communication with treatment care team providers, both medical and behavioral health, and within the community Incorporate health and wellness education and referrals for preventive care Respond to and incorporate physical health concerns into brief treatment interventions Provide crisis intervention and Warm Hand Off assessments to behavioral health staff Participate in weekly group and individual supervision, and clinical training seminars REQUIREMENTS: Masters in Social Work, in Marriage and Family Therapy, or in Mental Health Counseling Current New York State registration and license Bilingual in English and Spanish Recent clinical experience or training Excellent clinical knowledge with mentally ill and substance abusing population Knowledge of evidenced based treatment modalities and crisis management Excellent written and verbal communication skills Experience in working with lower socioeconomic and multicultural populations Ability to work collaboratively with other staff, treatment providers, and community agencies Highly organized, self directed, able to multi-task in a fast-paced environment Strong computer skills including working knowledge in Word and Excel, as well as capacity to navigate and enhance proficiency in these programs Patient centric with empathy, excellent active listening and communication skills, engaging and efficient Passionate about providing high quality care to low income, under-served residents and those lacking access to health care There are many fantastic benefits to a career at Open Door Family Medical Centers. Requirements

Candidates must hold a Master's degree in Social Work, Marriage and Family Therapy, or Mental Health Counseling and possess a current New York State license. Fluency in both English and Spanish is required, along with experience working with underserved and multicultural populations.
