Provide person-centered care to improve residents' independence and quality of life through individualized treatment plans and functional outcomes. Collaborate with an interdisciplinary team to evaluate physical conditions and recommend environmental modifications or adaptive equipment.
Requires an active and valid Texas Occupational Therapy license and must be in good standing with regulatory boards. Experience in Skilled Nursing Facilities (SNF) and proficiency with Net Health documentation are preferred.
