About the Role

The Palliative Care Home Visit Nurse Practitioner in Cartersville plays a critical role in delivering compassionate, patient-centered care to individuals with serious, chronic, or life-limiting illnesses within their homes.

This position focuses on managing symptoms, improving quality of life, and supporting both patients and their families through complex medical and emotional challenges.

The Nurse Practitioner will collaborate closely with interdisciplinary teams to develop and implement personalized care plans that align with patients' goals and values.

By providing expert clinical assessments, medication management, and education, this role ensures continuity of care and reduces unnecessary hospitalizations.

Ultimately, the Nurse Practitioner serves as a trusted healthcare provider who advocates for dignity, comfort, and holistic well-being in the home setting.

Minimum Qualifications

  • Current and unrestricted Nurse Practitioner license in the state of Georgia.
  • Master’s or Doctoral degree in Nursing from an accredited program.
  • Certification as a Nurse Practitioner (e.g., ANCC or AANP) with a focus on adult, gerontology, or family practice.
  • Minimum of two years clinical experience in palliative care, hospice, or related healthcare settings.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.

Preferred Qualifications

  • Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.
  • Experience conducting home visits and working within community-based healthcare models.
  • Familiarity with electronic health record (EHR) systems and telehealth technologies.

Responsibilities

  • Develop, implement, and adjust individualized palliative care plans in collaboration with patients, families, and healthcare teams.
  • Manage symptoms such as pain, nausea, and breathlessness through appropriate pharmacologic and non-pharmacologic interventions.
  • Provide education and counseling to patients and caregivers regarding disease progression, treatment options, and advance care planning.
  • Coordinate care with physicians, social workers, chaplains, and other interdisciplinary team members to ensure seamless service delivery.
  • Document all patient interactions, clinical findings, and care plans accurately in compliance with regulatory standards.
  • Respond promptly to patient and family concerns, providing crisis intervention and emotional support as needed.
  • Promote patient autonomy and advocate for patient preferences in all aspects of care.
